Translation update (fr)

- Use standard translation 'Catégorie' for 'namespace'
- Replace HTML entities by UTF-8 accented characters
- Minor fixes

French translation for autopage config

Add placeholder text to page name input

Fixes #88

Coding standards fixes throughout codebase.

Code layout and minor fixes for most settings.php files. Removed Italiam and Russian which were just copies of the English one.
