Fixed caching issue with ~~NOCACHE~~ in index.txt.

Made a signficant change - now puts the discussion pages under /talk:<namespace> rather 
than /<namespace>:discussion.  If you used the old one, be sure to move over all your
:discussion pages.

KNOWN BUGS
- if using indexmenu in sidebar, chache doesn't update when new pages are added or pages
  are deleted
