===== Installation =====

This plugin is part of the [[http://freemedforms.com|medical FreeMedForms project]].
Install the plugin using the [[plugin:plugin|Plugin Manager]] and the download URL above, which points to latest version of the plugin. Refer to [[:Plugins]] on how to install plugins manually.

===== Examples/Usage =====

Code:

Adds a complet blood pressure self-measurement form. Allow user to enter his blood pressure values, clear the form and send the form to the configurated email account (see configuration).

When user validate the form, a report is generated and sent to the configurated email. The report contains all the blood pressure values, all the averages and the main result. It is sent as a textual mail, including two attached files (an HTML version and the .txt file).
