In case of multiple inclusion levels in an inclusion hierarchy, the traversal
of it let to the content of the last visited leaf being prepended to the rendition
of the parent node thanks to a variable initialized at the wrong location in code.
Relocating the initialization of $this->doc to AFTER traversal fixes this problem.

Allow plugins to generate instruction lists ID dependently on included pages

Plugins like the discussion or the linkback plugin which use syntax command
to enable or disable certain functionality for pages during instruction set
generation so far executed these things in the wrong context of the
including page, not the included one, as the global variable $ID contained
the wrong page id.

This patch encloses the whole rendering process in the correct ID context,
opposite to the former XHTML-rendering only situation.
